Problem with the sites feature.

I have a problem adding a site using the sites feature in dolphin.  I am thinking it could be because I added that free simple blue template but not sure because it did work at one time.  Here is the error I am getting.  Anyone got any ideas how to fix this?  Is there a permission I am missing or something?  This is the error I get when I try to add a site.

Additionally, a 404 Not Found error was encountered while trying to use an ErrorDocument to handle the request.

i tested the original template and i stillget this error when trying to add a site.  i beleive the problem could be server related since i am getting the same error on my test site.  hopefully someone has an idea or has seen this before.

This is 100% hosting issues.  I got it resolved.
