Join form no longer works

I'm not sure why it doesn't work anymore.  I don't get any errors, even if the mandatory fields are not filled out.  It's almost like the join button just no longer works.

Any suggestions?

 I got impatient and did a site restore.  I don't want to post my site but I'll send you a link.  My join form works now but my splash is jacked up. :-/
