HTML or PHP Contact Form

Hi, has anybody got any coding or files of a Contact Form that corresponds with Boonex Site Style?

Thanks

Maybe be a bit more specific on what you want.  Dolphin comes with a contact form; "Contact Us".  Why not just add to it?

I am going to promote business accounts and need an external form so i can approve them and register the business account manually.

If you don't want to use the built-in contact form there are plenty of open source contact form scripts.  If you want one that will store results in a database, I recommend Form Tools.  I used Form Tools in the past and it is a very powerful script and can save results in a mySQL database.

It may take a bit of time to learn how to set it up but once you do, it is a wonderful tool for forms.  I was using it to take dinner reservations for a social group and the fact that the results are stored in a database made it easy to dump out the reservations (and the type of dinner ordered) when needed.

Form Tools is way beyond basic, it is a very powerful form processor, one of the best.  It can handle multi-page forms and payment processing.  In fact, it takes a bit of time to get an understanding of all it can do.  It has a great support forum for getting answers as well.  To use it all one does is create their form and Form Tools can parse it.  There is a for-pay module that will create and publish forms to one's website but it is not needed if you know how to create forms.  The fact that it stores the results in a database instead of just sending one an email is one of it's best features.  You can set up multiple users to handle the post-processing of the data received.

There is not going to be anything you can drop in that will mimic Dolphin.  You have to work at styling the application and incorporating it into your website.  If you can not do the work, I suggest you post a job for what you want to do and hire a developer to work at setting up the contact form you need.

According to your answer to my request, you were not looking for a join form replacement.  You can modify the join form all you wish.  You said you wanted a replacement for the contact form; join and contact are two different animals.

In order to get help, it is important for one to spell out in details what they wish to accomplish.

If you go to the market here and search for "join" you'll find several alternatives. Anton has a profiles splitter that will let you sperate business accounts from regular accounts. There is also an "advanced ajax join form" there.

Then you don't want an external join form.  You want a join form in Dolphin because it has to connect with the Dolphin Database unless you are going to add them manually.  In that case, any form processor will do, even the simple email ones.  Just embed it in an iframe on a site page you build.

Or, as was suggested, go to the market and look at what is listed there for join forms.
