Email Messaging Gone Wild

Upon a member joining,  my system is sending 2 copies of the email confirmation link at the same time. After approved & activated, it does not send profile change message.

That msg is where I tell the new member how to use the site & what its about so it is crucial to receive it.

I cant think of what may have caused this. The only thing I have done was to edit the email content with some new text.

Any ideas?
