Blank page shown on incorrect login

When an incorrect username or password are entered during login, a blank member.php page is displayed.

On some instances the following alert box message was displayed, but I can't consistently recreate it:

Error!
Your username or password was incorrect. Please try again.

Any idea how to fix this?

Thanks in advance!

Some additional, potentially useful information.

  • blank page is displayed when logging in from the home page (index.php), either using the login box on the page or the pop-up login box
  • alert box is displayed correctly (see attached) when logging in from the join page (join.php), both from the login box on the page or the pop-up login box

Also was able to capture this error using IE8:

More potentially useful information:

When logging in from member.php the incorrect username/password alert box is correctly displayed, both from the login box on the page and the pop-up login box

As a result, the problem seems to be limited to incorrect logins from index.php, while logins from join.php and member.php work correctly.
